GASafety is a subteam of a group of people dedicated to promoting just culture, and using it to learn from both success and less successful outcomes.
The FDR tool on this site is just part of our product offering.
Often, humans have a tendency to look at final actions in a sequence, and think backwards from there to reconstruct what happened and why.
The problem is, the 'Why' is not so easily explained. Reasoning backwards this way generally results in concluding 'the pilot should have done better' without establishing true cause.
Using a model for explaining human behaviour, including failure as well as success, GASafety and our associates specialises in looking further than the 'pointy end'.
The FDR tool is one way of recollecting a flight or an incident. This is the start -- from here, we look at what those involved were trying to achieve and what influenced their ability to succeed.
Only then, can true learning start that will benefit both those directly involved as well as those that are not.
